Do guys in like underwear ads and speedos stuff their crotches to make it look bigger?

All the guys in underwear ads seem to have these huge bulges, but then when I wear them they look really flat and a bit baggy. Do they stuff to make it look bigger or do they take the pictures while they have an erection or something?
